If you feel that your current home is unsuitable because of a medical condition and wish to be considered for medical priority when applying for housing, you should complete a self-assessment medical form.

Please be advised that you may be asked to complete a medical form when applying with These Homes. This form is not valid for Angus Housing Association but may be used for other landlords that you have applied with on These Homes.

Once you have completed the Tayside Medical Advisory Form, we will send this off to our independent medical assessor who will carry out an assessment of the information you have provided within the form. The Medical Advisory Service will then write to us with their decision. The outcome of their assessment will then be added as a priority pass to your These Homes Application. Please note that we cannot add medical priority to your application form without a medical assessment form being completed first.
